Futanari are distinct from transgender and intersex people in that they are entirely fictional and only follow the writer’s rules. They are nearly female in appearance except for having male reproductive anatomy. With their unusual genitals comes a high sex drive, which they use primarily on women and other futanari. They are usually dominant but some are submissive, or both.
